As defined by the Patient Protection and Affordable Care Act and the Secretary of the U.S. Department of Health and Human Services, the term includes: ambulatory patient services; emergency services; hospitalization; maternity and newborn care; mental health and substance use disorder services (including behavioral health treatment); rehabilitative and rehabilitative services and devices; lab services; preventive care and wellness services and chronic disease management; and pediatric services (including vision and oral care).
Evidence of Coverage —
This is issued in connection with Horizon BCBSNJ's Health Maintenance Organization (HMO) coverage and basically serves the same purpose as a certificate.
Exchange Cost Sharing Subsidy —
The share of costs covered by your insurance that you pay out of your own pocket. This term generally includes deductibles, coinsurance and copayments, or similar charges, but it doesn't include premiums, balance billing amounts for non-network doctors, facilities and other health care professionals, or the cost of non-covered services. Cost sharing in Medicaid and CHIP also includes premiums.
Exchange Premium Subsidy —
Refers to the money provided by a government entity as part of the Affordable Care Act.

A statement Horizon BCBSNJ sends a member describing how a claim was paid. It includes services provided, amount billed, payment made and any charges that are the member’s responsibility.
